If you want the Word version so you can edit it to suit your personal requirements,

oh I can edit a PDF easy enough
or change it to a .docx (word)

I had to copy that one of mine from a post
to Excel
from Excel to Word
from Word to PDF

I couldn't find the one I had started to modify - but I like yours..................

The conversion though can sometimes ruin the table format (can't decode the cells properly) or the columns.
Let me know if you want the originals.
